In my opinion, targeting the availability of clean freshwater and the adequate disposal of waste water in the developing world would have the biggest cumulative health impact. By providing a safe water infrastructure in the availability and the disposal, many breading grounds for vectors will be destroyed and the overall health situation of many people would be improved.
